At SHARP Treatment of South Bay, our doctors have managed over a thousand personal injury cases. Our mission is to properly diagnose, treat, and rehabilitate our patients to pre-injury status. Our office will be happy to accept patients on a lien basis, and in cases where funds become limited, our aim is for all parties to reach a fair and equitable disbursement.

Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
